Chronic Disease Management Programme (CDMP) in Singapore

The Chronic Disease Management Programme, or CDMP, was introduced in 2006 to provide the option for patients to draw on their MediSave to help reduce out-of-pocket payments for outpatient treatment required in the management of their chronic diseases. Over the years, the government continued to fine-tune the programme. The 20 chronic diseases that are covered in this programme are:

To ensure prudent use of MediSave funds, two safeguards are in place under the CDMP:

a) Co-payment: A co-payment of 15% will apply to each outpatient CDMP bill; and

b) Annual withdrawal limit: An annual withdrawal limit of $500/700 per patient applies. This will be reset on 1 January of each year.


Example: For a CDMP bill of $100, the patient pays $15 out-of-pocket. The remaining $85 can be claimed from MediSave.


From 1 Feb 2024, patients seeking CDMP treatment at their enrolled Healthier SG clinics will have the 15% co-payment waived. They can use MediSave to fully pay for their CDMP bills, up to the relevant annual limits under the MediSave 500/700 scheme.


How can I participate in the CDMP?


To qualify for MediSave 700, patients would have to be under the Chronic Disease Management Programme (CDMP) to claim up to $700 yearly for chronic disease-related treatments, vaccination, and screening cost.


For Singapore residents aged 60 and above, on top of the MediSave 500 and MediSave 700, there is an additional $400 annual limit given under the Flexi-MediSave scheme. The scheme also allows for individuals to pay for their spouse’s outpatient fees. Flexi-MediSave can be used at participating CHAS Medical GP clinics, polyclinics, and specialist outpatient clinics.


Each MediSave account holder will need to sign a Medical Claims Authorisation Form (MCAF) to authorise the CPF Board to deduct his/her MediSave funds for the treatment of the patient.


What is the Medical Claims Authorisation Form (MCAF)?


You can authorise the medical institutions to deduct from your MediSave, claim from MediShield Life, and/or your IP for medical expenses that you and/or your loved ones incur using the MCAF.
